The following brochures, forms, and necessary information for the processing of workers' compensation claims in the state of Utah are available from the Industrial Accidents Division and are itemized with their costs as follows: A check for requested items on the form must be received by the Labor Commission before shipping.

Workers' Compensation Poster - English Spanish
No charge for posters, however, there is a shipping cost of $4.00 for postage on 25 or more posters. Orders over 100 will have to be reproduced by the companies and they must use the Utah labor Commission information and format. Unlimited amounts if you come in and pick up.

Employee's Guide to Workers' Compensation On-Line English Spanish
You can order a limit of 50 brochures without mailing cost. Orders of 50 - 100 brochures will require $4.00 for postage. Large orders will have to be reproduced by individual insurance companies or self-insured employers using the Utah Labor Commission information and format. Unlimited amounts if you come in and pick up.
